Links for CARA Report Instructions and Forms

Postby Jane » Fri Jun 24, 2011 2:07 pm

Attached please find the new CARA Report procedure and forms as provided by Holt’s corporate office. I confirmed the process has been streamlined significantly in that we no longer need to have a Social Worker notarize our CARA reports. In addition, no notarization is needed with the family signature on the CARA reports. (Basically, families complete the CARA report as usual. Once signed the report is sent to Holt’s Eugene office where Dean will sign the report, Mary will notarize and copies will be communicated to India.)

One item of note, it was requested to stress families please include a photo ‘cut to size’ on the top page of the CARA form. Editing and affixing your photo reflecting your child’s face not to exceed the 2x2” size provided will ensure timeliness and is ‘greatly appreciated’.

Cheers~
Jane

Question Jane:

In the instructions to which you linked above, it DOES say the family signatures need to be notarized (in the first paragraph under important reminders, near the end of the 2nd page). Clarification??

Blessings,

Reached out to Holt, please see attached response:
Basically, families just fill out the CARA Report as usual- still requires family signature but not notarization and then send it to Holt. Dean then signs the report, I notarize his signature, make all the copies (I have to make 5 copies to send to varies Indian officials) and send to India.

Thanks for the catch Sandwich... loving your siggie, puts a smile on my face every time. :)
Cheers~

Ok, great! Thanks Jane, for the follow up. We have one coming due in Aug. Thankfully we're down to the 6 month reports. And this Aug. makes 4 years. I think we're on the 5-year plan, so only 2 more after that! Yay!

BLessings,

These links are fantastic! Thank you so much :D I'm just wondering how many photos we attach to the report (aside from the one on the first page). In the past I've included 6-8 for each report and affixed them to white paper which I thought was the standard protocol. Is that correct? From the instructions it seems these just get sent to VCT, BSSK or CWB and not the officials. Do the rest of you send photos?

Hello Mom2Bharat!
It’s good to hear from you! I hope you are well. Re: photo's, aside from the photo on the front and actual report I include exactly what you noted. I typically include 6 to 10 photo’s in addition to the required report. I’ve heard of others printing 2 or more photos on a single page of paper, vs. affixing them to the top. This is completely acceptable per Holt. Also, I make copies of my photos in triplicate, one for CARA/VCT/Foster Family. I want to give VCT and DD’s foster family an update as they are a special extended part of our family.
Cheers!
